It has become in-vogue for insurance “providers” to contract with a third party company to review (i.e. deny) radiologic studies, such as MRIs, for medical necessity and appropriateness. If they deem that such a study is “not medically necessary,” the process for a physician and patient to overcome it is purposefully onerous and requires a disrespectfully time-wasting amount of phone calls and paperwork.

Approximately one month ago, I saw a 40’s year-old-woman who’s main complaint was knee pain and decreased range of motion. On exam, there was a weird tender lump jutting out from the side of her knee (what I, in medical parlance, described as a “lateral joint deformity”), and I detected some degree of possible meniscal instability on manipulation of the joint. All of this would lead most people with medical training, and some without, to come to the conclusion that an MRI of the knee to specifically examine the soft tissue structures is the appropriate next diagnostic step.

What follows is the phone conversation I had earlier with the third party company involved after fighting with them for four weeks, and subsequently being denied, for the test:

[called  main number, on-hold for 16 minutes]

Operator: “Hello, this is xxx, may I have your patient’s account number please.”

Me: “Hi, this is Dr. John Cmar at Sinai Hospital of Baltimore, how are you doing this evening?”

Operator: “Um… fine. You patient’s account number, please.”

Me: [account number given] “I’m calling to appeal a decision for coverage on an MRI for a patient of mine.”

[information is exchanged]

Operator: “I’ll need to locate a Medical Account Manager for you. Please hold for the physician.”

[on-hold for 22 minutes]

Operator: “Um, I’m sorry sir, but it appears that we no longer service insurance company yyyy. I can’t help you.”

Me: “Really? But I’ve been dealing with you for four weeks. When did this change occur?”

Operator: “November 1st.”

Me: “But I have a letter here from you issuing a final denial on November 4.”

Operator: “Yes… I’m sorry, but I can’t explain it. It makes no sense.”

Me: “I agree. So, what’s my next step? Start from square one and four weeks ago, and issue another referral?”

Operator: “That’s as good an idea as any. I really don’t know. I’m sorry.”

Me: “I’m sure you understand this isn’t satisfactory. Do you have a manager I can speak with?”

Operator: “Sure. Please hold so that I can locate a manager for you.”

[on-hold for 18 minutes]

Operator: “I’m sorry sir, but there’s no manager available right now.”

Me: “… really?”

Operator: “Yes, sir, I’m very sorry.”

Me: “Well… thanks for your help, then. I’ll reissue the referral to the insurance company.”

Operator: “Is there anything else I can help you with today?”

Me: “You… have got to be kidding.”

From a purely practical standpoint, that was from ~6pm to 7pm wasted on fighting futily for coverage of a medically indicated study for one patient, that has been previously delayed for four weeks and will continue to be delayed further. For me as a salaried hospital employee, that’s a personal debt in time and energy… but for a private practitioner, that would be a direct loss of livelihood, and for the patient, an inexcusable delay in specific diagnosis and accurate treatment.

I am thankful that it only took an hour of my time. Oddly, however, I happen to have more than one patient.

Weird science!

My pre-auth for the next rounds of frankenbabying went through today.

I'm covered for four rounds of injectibles/IUI. Although my cost for drugs is $$$$$ because my bennies are by percentage, not flat copay. But still.

Holy crap, this is really happening.

A VERY SEKRIT PASSPHRASE

There were 38 independent bookshops around the land who had Graveyard Book parties. The people at Harpers somehow got it down to 11, and they sent them to me to judge the winner. The winner gets me for a signing in December. I watched the 11 videos/descriptions/ photos. I watched them again. I watched them yet again, this time with Lorraine, my assistant, watching too and saying helpful things like, "They are all so good. Whoo. Don't know how you'll make a decision. Look at that! They're line dancing to Monster Mash! And that Death is on stilts, isn't he. Is that a horse? A horse in a store? These are amazing." The fourth time, Woodsman Hans wandered in from the deep woods (where he is making a pond) and watched them too.

Then I made my decision. I called Elyse Marshall at Harpers and told her. "Ah," she said. "I'll have to check with the lawyers to find out if you can do that."

So we wait.

Today it occurred to me that in the past when I've had friends on tour, I've often done special "Neil sent me" things, where people who come from this blog get some special free thing, which a) is nice for the people who get the free thing and b) tells the person on tour that people are really coming from the blog. I did it with Thea Gilmore (who is starting a new UK tour next week. People in the UK, go and see live Thea Gilmore, for she is wonderful: http://www.theagilmore.net for dates and venues.) I've done it for The Magnetic Fields, who, incidentally, have a new album coming out on Jan 26th. And then there's the Green Goddess restaurant in New Orleans, where you can mention the "Mezze of Destruction" to tell them you came from here and get sent something wonderful to eat or drink. (It changes, depending on what chef Chris DeBarr feels like making.)

I should do it for Amanda. I called her up and told her.

She called me back. "Beth and I have put our heads together and come up with a code phrase for people from your blog," she said. "So they say it and get a special free thing from the merch table."

"Fire away," I said.

"We think they should come over to the merch table and point to this poster...




...and say 'That chick in the yellow corset crowdsurfing looks kind of hot. I wonder if she's dating anyone?' And then they get something for free."

I said I thought that was a very bad idea, because people might say that anyway, and it was an awful lot for people to remember. And what if they sold out of that poster early that night?

I said, "What about any variant of 'Neil sent me from his blog?'"

"Absolutely not," she said. "That's boring."

I told her to leave it with me.

And then I stared at this screen glumly, with nothing happening in my head, and real work I should be doing starting to nip at my heels. So I turned to the Oracular Orb of truth at http://www.neilgaiman.com/oracle/ and I clicked on the orb and shook it.

I am a long-time fan, and have even met you backstage at a Tori show (though that was many years ago!). I am writing to ask a bit of a favor.

About 10 years ago, I appeared on 20/20 with Tori, speaking about sexual violence. Since then, I've stayed close with Tori whose been a mentor of the best kind. I also started a nonprofit, Pandora's Project, that provides support, information, and resources to rape and sexual abuse survivors and their supporters. We operate Pandora's Aquarium, an online support group with more than 20,000 registered members.

Recently, I was named a 2009 L'Oreal Woman of Worth for my volunteer work with Pandora's. I was chosen for this honor from more than 2,500 applicants.

Now, one of the ten 2009 Honorees will be selected as the national honoree through a public online vote. Her cause will get an additional $25,000, and a lot of media exposure. This is the first time L'Oreal has recognized a sexual violence organization, and becoming the national honoree would allow me to shine a spotlight on this issue that affects so many women and women.

Your correspondent asks "Will you be reading the original version where the wolf actually is killed, and not the 'oh my goodness our kids can't hear about death' version in which they bring him to the zoo?"

I fear she's in error; in the original version, written by Prokofiev, Peter snares the wolf, then convinces the hunters NOT to kill it, but to take it to the zoo.


I've been researching, and that's what I found out too. Wikipedia has a list of changes made in various versions of the story (Disney, for example, had the wolf not eat the duck). But the wolf was always taken to the zoo...

So there was this couple in Colorado. They decided that they wanted to be famous. So they faked their son being carried away in a helium balloon. There are two things that make me go WTF about this:
1) What were you hoping that this kind of publicity would get you? Even people have stuff like this legitimately happen to them don't stay popular very long.
2) It might have been less discoverable as a fake if your son had actually been in the balloon. Clearly you were not desperate enough.


In other non-news, dream about running around in a gymnasium trying to avoid falling fire and jump through brightly-colored teleport portals which were somehow keyed from nearby paper towel dispensers, but failing to actually do so. I blame WoW.

Additions and improvements for Paid Feature

We've made some additions and improvements to Notes!

The Notes feature has been added to two action-taking pages:
  • You can now add a Note directly on the Add a friend page - handy if you'd like to mark down where you met them or another name you know them by!
  • On the Ban and unban users page (under Account -> Privacy) you can now add a Note, including to a group of users all banned at the same time (so that next year you won't need to ask yourself "hey, why did I ban these guys?")

Other changes:
  • When you're viewing your existing Notes they're grayed out; click in a field to activate it to change the text (this page can be found from the header by using Profile -> Manage Notes)
  • Changes to editing:
    • When you're going to create a new Note but one already exists, you'll get a warning that you're editing an existing Note
    • You can now delete a note from the "Edit note" pop-up in the hover menu
    • You can now delete notes for multiple selected users on the Manage notes page
    • When you change Notes on "Ban|unban users" page, they can be edited and saved with "Save Changes" button

LiveJournal Major Notes: Notes, Tweaks, Bug Kills, LJ_Cares!

Notes augmented

We've enhanced and de-bugged Notes. If you haven't tried it yet, now's the time! You can create a private note when you ban multiple users. You can also delete multiple notes at once. Lastly, paid users have the option to add a note (visible only to you) whenever you add or remove a friend (guaranteed to avoid embarrassing social mishaps). If you don't currently have a paid account, you can upgrade now! It only takes a few minutes and costs less than a bad shopping mall haircut (plus, it's way more fashionable)!

Product tweaks and bug kill

  1. In another effort to zap spam, comments containing links from domains LiveJournal deems untrustworthy are now automatically screened
  2. If you sign up to get notifications of the Writer's Block question of the day, you'll now see the daily question in the email notification, so you'll have a little extra time to ponder before you post. You can subscribe to Writers Block notifications here
  3. The issue causing random comments to vanish has been fixed!
  4. If you visit a LiveJournal page and get prompted to log in, you'll be returned to the same page after you sign in (Thanks, Dreamwidth)!
  5. If you don't edit the timestamp for an entry at all, the entry timestamp will indicate the time the entry was posted instead of the time the Update Journal page was loaded
  6. Comments with paddings/backgrounds render correctly within the comment box (and will no longer wrap outside the box and break frames/margins)

New FCK fixes rich text editor!

  1. We've updated our RTE (Rich Text Editor) to FCKeditor version 2.6.5
  2. When switching from the RTE to HTML editor, links for syndicated feeds are no longer broken
  3. RTE now functions properly in Safari 4.0
  4. An extra line/space will not be auto-inserted whenever you switch from RTE to HTML editor
  5. The insert image link now works correctly in all browsers

The Triple Undisputed Burger

After two hours, 15 minutes and two Marlboro reds, Schreiber declared himself the conqueror of the Triple Undisputed. Paschalis and I pretended not to notice the uneaten top bun and the bits of meat hidden beneath it. After all, Schreiber had done something neither of us would ever do.

Dream: Me, the Presidential trophy wife

I had the most awesome dream last night. It was vivid, had solid dream logic, and featured me as me, with my actual life and perspective rather than a sort of partial or totally different dream-me.

I have a 3rd cousin in Germany who we've visited a few times. In this dream, her family owned a small palace, of ~18th century design. President Obama had decided that he needed to use this palace for some sort of diplomatic purpose. So he decided to propose marriage to me. The newspapers were filled with analyses of Obama's new policy of negotiating treaties as a monarch rather than a President, including sealing treaties with political marriages.

This union was negotiated almost entirely by his entourage. I didn't speak the President much directly, and when I did, he was with a half-dozen other people. The union was intended to be short-term and entirely political, and it had no effect on either Michelle or Studentbane. We would not need to divorce our actual spouses. However, the four of us: him, me, Michelle, and SB would be living together in the palace for a few months out of the year.

So I sat down with SB, and we decided that we might as well go for it. I wondered what insights the personal contact with the President would give me. And I had no idea whether there would be any expectation to actually consummate this marriage. Being that it was short-term, I figured I'd roll with however things fell out. I was amused that sex played no part in the negotiations. I idly wondered what the big guy would like like in the sack, but my dream self actively didn't seem to care one way or the other.

SB and I toured the palace, which was quite small, very ornate, and featured a cafe on the top floor. Then the alarm went off and I woke up. Damn. I'd've at least liked to see what kind of marriage ceremony this would be. I think the dream didn't know what to do with that either and thus kept stalling :).

All week, I've been, to my knowledge, sleeping soundly through the night. Then I wake up and feel like I didn't sleep at all. I feel like I'm wandering through a haze. SB is having trouble with a catch in his back that won't go away. I'm starting to wonder if there's some issue with our mattress. I think we're going to have to spend a night on the guest futon for experimentation. The mattress certainly doesn't FEEL bad.

Oops, so that's where the last two hours went

Thanks to [info]sorakirei, I checked out something called Horrible Turn, which basically is fanfiction, except it's more like A ONE-HOUR WELL-CRAFTED MOVIE MUSICAL fanfiction, a prequel to Dr. Horrible etc etc. It's actually pretty good, but I've barely seen anything by Joss Whedon EXCEPT Dr. Horrible, which I've only seen like twice, so maybe I don't know how to judge this stuff, but what I do know is that it certainly LOOKS lovely to me.

Anyway, I dunno, it's been an evening. I had sushi for dinner and came home and should be writing entries about school but for whatever reason I seem to be incapable of writing about school at home, and find other things to be distracted with instead.

Also it's COLD out now.

And I got my JLPT voucher today and it makes me feel like a dipshit for not studying more.

When a US studio snaps up remake rights to a foreign film, it’s often hard to picture the elements that made the original special in the first place surviving the translation into the Hollywood style. The piece might depend on cultural specifics that won’t work with American characters. It might succeed on the basis of a delicately sustained tone that seems impossible to capture a second time. The impending Hollywood version of Let the Right One In comes to mind. For similar reasons I've always been glad that the long-bruited American version of The Killer never made it out of development.

Overheard [HK, Felix Chong and Alan Mak] is, on the other hand, the kind of movie Hollywood should be remaking. It’s a plot-driven thriller with a fresh premise that could occur in any country with a stock market. The film makes diverting hay of its basic concept but doesn’t knock it out of the park, leaving room for the hypothetical remake writer to improve on the original.

Three underpaid cops from an electronic surveillance division enter a slippery slope of danger and corruption when they decide to cash in on the insider trading scheme they’ve been assigned to listen in on. Lau Ching Wan downshifts his charisma into a lower key as a passive guy secretly seeing getting another chance with his supervisor’s estranged wife. Daniel Wu is a young officer pressured by his rich fiancee’s father to increase his earning potential. The truly memorable performance comes from Louis Koo, who deglamorizes to play a disaffected working-class schlub facing a family medical crisis.

Of structural interest is the way that the film shifts through different cop sub-genres for each of its three acts. It starts as a stylish techno-procedural, becomes a noirish guilt spiral in the middle, and then rounds its final turns in typically doom-laden HK fashion.

Ragged in spots and perhaps failing to wring maximum juice from its original core idea, it’s still worth a gander if you like the actors or are a diehard Hong Kong cinema fan. Like most HK movies you can safely assume that it will show up as an import DVD in fairly short order.



I caught this at the gala opening of the Reel Asian Film Festival, which meant that it was proceeded by a punishing twenty minutes of welcoming speeches. Yikes!
